Adult Head Injury Program

The Adult Head Injury Program provides assistance to Missouri residents who have sustained a traumatic brain injury (TBI). Through a contract with the Missouri Department of Health and Senior Services (DHSS), Bureau of Special Health Care Need the Cole County Health Department (CCHD) can assist individuals with TBI's and their families with locating, coordinating and purchasing rehabilitation and psychological services.

Program Eligibility Requirements
  • Between 21 and 65 years of age
  • Missouri resident
Service Coordination
Service Coordination is available to adult survivors of TBI regardless of financial status. All individuals will receive an evaluation and assessment of needs. They are also provided information and education regarding the causes and effects of TBI and prevention of secondary clinical conditions. A service plan will be developed to assist the individual with their needs.
Service Coordination Services include:
  • Assistance in locating and accessing resources including but not limited to:
    • Cognitive/behavior training
    • Counseling
    • Housing
    • Medical care
    • Rehabilitation
    • Transportation
    • Vocational training
  • Regular progress evaluation
  • Ongoing updates to the service plan
Rehabilitation services are available to individuals who are eligible for the service and whose income is at or below 185% of Federal Poverty Guidelines. Rehabilitation services may be provided when necessary to facilitate the participant's achievement of a long-term goal as indicated in the SHCN Service Plan. The provision of rehabilitation services is subject to the availability of funds which are appropriated annually. All rehabilitation services are required to be pre-authorized. The Department of Health and Senior Services is a payer of last resort, so the Adult Head Injury service coordinator will assist the client to apply for any other payment resources before submitting requests for use of SHCN funds.

The following Rehabilitation Services are funding by the Adult Head Injury Program:
  • Adjustment Counseling - Up to 26 sessions in lifetime
  • Neuropsychological Evaluation and Consultation
  • Pre-Vocational/Pre-Employment Training - Up to 45 hours in lifetime
  • Transitional Home and Community Support - Up to 1,000 hours in lifetime
